New Patient Safety Section Created for Health Care Providers
Providers now have a single point of access for information and tools to promote a culture of safety when providing care.
The new section features information and resources related to two key areas:
- Patient Safety Alerts; and
- Medication Safety.
Patient Safety Alerts focus on sharing information about a critical incident or event, along with recommendations to prevent the event from recurring. These alerts also promote learning, and help to improve health system quality and safety.
Medication Reconciliation is a formal process in which healthcare providers work together with patients, families and care providers to ensure consistent communication of medication information across patient transitions of care.
